Admission Process: 1. Call for a brief, screening interview: Men: 609-345-4035 ext. 209, Fax# 609-345-0952 Women: 609-345-2433 ext.130 Fax# 609-345-1112. Whenever it is possible the person seeking treatment should be available to participate in the phone call. At that time you will be informed regarding bed space and initial eligibility. 2. Applicants on methadone must be referred by their methadone treatment program. Clients on dosages above 125mg. will need a written explanation from the clinic Doctor explaining the necessity for the dose level i.e., detail of the clients usage history, the client is on a medication, which decreases the potency of the methadone etc. 3. Prior to setting an admission date we will need the following information (fax): • Medical Status- TB test results (within 6 mos.), presently prescribed medications, recent injuries, most recent physical exam results, information regarding any medical condition that can interfere with an ability to fully participate in treatment, chronic illness that require attention or other physical problem that requires ongoing care i.e. physical rehabilitation. • Psychiatric Status- If applicable, results of recent evaluation, a list of prescribed medications, and history of any hospitalizations. • Legal Status- Information regarding pending charges and ongoing cases either criminal or civil that require court appearances during the course of treatment. 4. The admission packet will be reviewed within five working days and you will receive a call to schedule an admission date or to discuss our concerns regarding eligibility. 5. Whenever possible the admission date will be within a week of approval. 6. The client will need to arrange their transportation to IHD. With prior notice we can arrange for pick up at the Atlantic City bus or train terminals. Admission appointments will be between 8:30AM and 2:30 PM. Individuals on methadone will be scheduled before 10 AM. If an individual is unable to arrive during this time frame different arrangements will be made on a case-by-case basis. 7. When arriving for admission please have the following items: • Legal Identification – one of the following; birth certificate, driver’s license, passport, county I.D. and SS card or number (picture ID preferred). • Medicaid or other insurance card (if applicable). • Prescribed medication in original container. • All personal items for your stay (see attached list). • Names, addresses and phone numbers of anyone we need to contact regarding your treatment. Individuals are considered ineligible for any of the following reasons: The client is a sexual offender (Megan’s Law registration required). The client has been classified as an arsonist and/or history of arson within the past ten years. An incident of physical assault within the past 18 months. The client has scheduled court appearances as a result of open/active warrants or an ongoing case. That cannot be postponed until treatment is completed. This does not include drug court. The client has medical or dental conditions beyond the scope of our medical staff that requires ongoing outside medical care. This does not include HIV related care. Clients must be fully ambulatory and be physically able to participate in our on site work assignments and focus on our treatment program. The client’s psychiatric condition is such that: 1. Is a safety risk to him/herself or others; excessive impulsivity coupled with anger, aggressive behavior and/or acute depression. 2. Based on client’s history or a professional assessment, it is unlikely that the individual would be employable despite psychiatric medication and addiction treatment. 3. The client has not been stabilized on medication. 4. The client’s psychiatric problems cannot be managed by the in house IHD staff. 6. The client has attempted suicide within the past year. If any of the above mentioned circumstances have been or expect to be resolved in the immediate future we are open to a closer review of the application. Please explain these circumstances in a cover letter. Items to Bring: 2 Bath towels and face clothes 7 changes of clothes 1 coat or jacket 7 change of underwear 10 pair of socks 1 pair of pajamas and a bathrobe 1 pair of slippers & shower shoes clock or clock radio Rain jacket 4 pr. sneakers , shoes Eyeglasses Basic hygiene needs, i.e., toothpaste, deodorant, toothpaste, soap, comb, lotion, hair-brush Any money will be placed and maintained in the client’s account Note: We do not have storage space for surplus clothes and will not be responsible for jewelry and other valuables. Clients with more clothes or other items than allowed under this policy will be asked to have their extra items sent home within 72 hours from the time of admission. Items not allowed: Cell Phones, Bank Cards, Food Items, drugs & drug paraphernalia, cd players, magazines or books of a sexual nature. Do not have any magazine subscriptions forwarded. |
